One 6 × 4 in label per Letter sheet (a bulk run gives one sheet per batch). Print at 100% scale (no "fit to page" — it shrinks the QR's quiet zone), trim on the faint guide, tape it to the bag. Scanning the QR opens Label Verify for that batch.
